Parent and Family Forms
For the safety of our passengers, an Emergency Information Form should be completed for all passengers that use the Chelsea School District Transportation System. An Alternate Transportation Request Form should also be completed for any student who is dropped off at a location other than their own home address.
Permission from BOTH LOCATIONS - parent requesting student to be transported to a location other than their home address and the receiving adult from new location must be given to the Transportation Office prior to a student being dropped off at the new location.
Forms are located below and should be faxed or emailed to the Transportation Office 3 days prior to service start date.
