Parent and Family Forms
For the safety of all passengers, an Emergency Information Form must be completed for every student who uses the Chelsea School District Transportation System.
For the 2025–2026 school year, only riders with changes to their transportation needs from previous years are asked to complete a Transportation Registration Form. This includes new students, students who have moved, or those needing an alternate drop-off or pick-up location. The form includes an optional section for alternate addresses—no separate request form is needed. If an alternate location is listed, permission from both the parent/guardian and the receiving adult is required. Forms must be submitted at least 3 days before service begins.
